Claims Form

On August 10, 2009, the Court granted final approval of the Class Action Settlement, and authorized disbursal of the settlement funds. Settlement Checks are scheduled to be mailed to class members on approximately October 20, 2009. Claims Period has Ended, and we are no longer accepting Claim forms
